Donna Altshul, RDH, BS: Dental Hygienist & Dental Program Director A native of Schenectady, New York, Donna received her Associate’s degree in Dental Hygiene from Hudson Valley Community College as well as her Bachelor’s degree in Management from SUNY Empire State College. Donna joined WHCHC in 2008, bringing with her a myriad of experiences. As a champion for dental health, Donna has worked in private practice, a school sealant program, a dental program in a community health center, and a hospital-based dental residency/dental program. Donna has also worked with the NYS Department of Health with the Bureau of Dental Health. When not in the office, Donna enjoys being outdoors and exploring the beautiful island while hiking, snorkeling, camping, and through meditation and healing practices.
Kyoko Awamura, DDS, MPH, MSD: Pediatric Dentist Dr. Awamura was born in Nagasaki, Japan and first came to the United States as a foreign exchange student at the age of 17. She received her undergraduate degree from Eastern Washington University and earned her Masters of Science in Pediatric Dentistry, her Masters in Public Health, as well as her DDS from the University of Washington in Seattle. She also completed her pediatric dental residency at the University of Washington and Seattle Children’s Hospital . After graduating, Dr. Awamura started her own private pediatric dental practice in Bellevue, Washington. She also served as a Clinical Professor of Pediatric Dentistry at the University of Washington. In 2011, she moved to Big Island and started working for West Hawaii Community Health Center. Currently, she is a consultant to the American Board of Pediatric Dentistry and is an officer of the College of Diplomates of the American Board of Pediatric Dentistry. In her free time, Dr. Awaramura enjoys snorkeling and yoga. She is also busy embracing the local culture by practicing and performing hula with a halau in town.
Dr. John A. Gawlik, DMD, MHA: Pediatric Dentist, Dental Director Dr. Gawlik was born and raised in New Jersey, where he received his undergraduate degree from Rutgers University while on a 4-year Army ROTC Scholarship. He then completed dental school at the University of Pennsylvania in Philadelphia. While serving as a dentist in the US Army, Dr. Gawlik completed a one-year General Dentistry Residency, two years of Advanced Training in Pediatric Dentistry, and a Master in Healthcare Administration from Baylor University. His 22-year Army career took him to Oahu, Italy, Germany, and several Mainland locations. After retiring from the Army in 2006, Dr. Gawlik worked in private practice in the Phoenix area until 2013, and then in community health on California’s Central Coast for several years before returning to Hawaii in 2017, where he is especially enjoying teaching pediatric dentistry to the NYU-Lutheran pediatric dental residents. Dr. Gawlik and his wife, who met in high school band, now play in the West Hawaii County Band. He also enjoys running, hiking, scuba diving, and paddling with the Keauhou Canoe Club. He and his wife have two successful grown sons living on the Mainland.
Rachael Hawkins, MSW, LCSW: Licensed Clinical Social Worker LCSW Hawkins was born in Sunny Valley, California and received her undergraduate degree from Sonoma State University. She received her Master of Social Work from San Jose State University and worked as a Clinician in California before moving to Hawaii in 2007. LCSW Hawkins served as a School Based Behavioral Health Specialist in the Department of Education in West Hawaii and began working with West Hawaii Community Health Center in 2013 as a Behavioral Health Clinician. LCSW Hawkins loves being outside everyday, and running and hiking with her dog.
Daisy Kim, DDS: General Dentist Dr. Kim was born in Honolulu, Hawaii and grew up in Southern California. After completing her undergraduate education at the University of California in San Diego (UCSD), Dr. Kim received her dental degree from the University of Southern California. After dental school, she served in the US Army as a general dentist followed by a five-year-period in Alaska, traveling to remote villages to care for Alaskan natives and local residents. In Summer 2015, Dr. Kim joined West Hawaii Community Health Center as a general dentist. She enjoys spending time outdoors with her dog and taking part in new cultural experiences.
Diane Logan, PhD: Clinical Psychologist Dr. Logan was born in Youngstown, Ohio and received her undergraduate degree at The Evergreen State College in Olympia, WA. She earned both her MS in Psychology and PhD in Clinical Psychology from the University of Washington in Seattle. Her clinical experience includes residency at the Newington VA and a community health center in Connecticut, and postdoctoral clinical training at the Providence VA and an outpatient center in Rhode Island. Dr. Logan completed a two-year postdoctoral fellowship at the Center for Alcohol and Addiction Studies at Brown University, and has published numerous peer-reviewed articles and provided clinical trainings across the country on substance use disorder interventions. Dr. Logan joined WHCHC in 2015, and enjoys the outdoors, running, watching football, and spending time with her family and their two dogs.
Heather Miner, MD: Family Physician Dr. Miner, a recent family medicine graduate, returned home to Kailua-Kona in August 2013. Dr. Miner attended Santa Clara University in California and received her MD at the University of Hawaii. She completed her medical residency at Ventura County Medical Center in California. Dr. Miner loves to spend time with her family, enjoys surfing, and loves her two rescue dogs dearly.
Susan Smith, RDH, BA: Dental Hygienist Born in Fresno, California, Susan attended Fresno City College, where she received her Associate of Science degree in Dental Hygiene. Afterwards, she received her Bachelor of Arts degree in Management and Organization Development from Fresno Pacific University. Before joining WHCHC in 2011, Susan worked in private general practices as well as periodontal specialty offices. In her spare time, Susan enjoys international travel, paddling with Keauhou Conoe Club, swimming, snorkeling, scuba diving, reading, and gardening.
Mary Ann Terminello, PsyD: Clinical Psychologist Dr. Terminello was born in New York and received her dual-undergraduate degrees in English and Psychology from the University of Northern Colorado. She earned her MA in Clinical Psychology from the University of Hartford, and became a Clinical Psychologist after receiving another MA and a PsyD from Argosy University in Tampa, Florida. Dr. Terminello served as the Mental Health Care Coordinator at the Department of Health Hawaii Family Guidance Center in Honolulu, as a therapist at Devereux Cleo Wallace in Colorado, and also as a Care Coordinator for System of Care with the Menta Group in Illinois. Dr. Terminello began working at West Hawaii Community Health Center in 2009 and likes spending time with family and friends, the beach, and the outdoors.
Ammon Whigham, RDH: Dental Hygienist Ammon joined WHCHC in January 2016 as a registed Dental Hygienist. He considers Alabama, Utah, Maine, and Hawaii home. Ammon received his Associate of Science degree in Dental Hygiene at University of Maine Augusta in Bangor, and expects to receive his Bachelor’s degree in May 2016. Before arriving at WHCHC, Ammon worked at Tropical Smiles Dental in Kona as a Dental Hygienist. In his spare time, he enjoys reading, music, going to the beach, spending time outdoors, and hanging out with friends and family. Ammon also has a deep appreciation for all things Hawaiian.
Kelly Nance, Dental Hygienist Kelly was born in Los Angeles, California. She received her associate of science in dental hygiene from Oxnard Community College in California. She practiced in a general dental office and a periodontal dental office in Southern California. She has spent the last six years in rural Alaska providing preventive and hygiene services to the Alaskan Native population. Kelly has been traveling to Hawaii since 1988 and is very happy to, finally, call Kona home. She likes to explore the island, spend time with her family, and paint.
